Can PCOS start or worsen in your 30s even if you didn't have symptoms as a teenager?

Can thin women have PCOS?
Yes. PCOS occurs in women across the full range of body sizes, sometimes referred to as "lean PCOS" when it occurs in a woman who is not overweight. Insulin resistance can be present even in a woman who is not visibly overweight, and the hormonal and cycle-related symptoms of PCOS are not limited to any particular body size. Weight is one factor that can worsen the underlying pattern, not a requirement for the diagnosis itself.

Is inositol the same as Metformin?
No. Metformin is a prescription medication, while inositol (specifically myo-inositol and D-chiro-inositol) is a naturally occurring compound available as a supplement. Both improve insulin sensitivity through different mechanisms, and research suggests inositol combinations can produce meaningful improvement in insulin sensitivity and ovulatory function for many women with PCOS, sometimes as an alternative to Metformin and sometimes alongside it, depending on individual response and preference.
Can PCOS symptoms come and go, or do they stay constant?
Symptoms can genuinely fluctuate over time, worsening during periods of higher stress, significant weight gain, or major life changes, and improving somewhat during periods of better sleep, lower stress, and consistent nutrition and movement. This fluctuation does not mean the underlying condition has resolved or returned. Is PCOS genetic? Should I worry about my daughter having it too?

What blood tests are wrong or misleading if I just get a standard annual physical?
A standard annual physical typically includes a basic metabolic panel and sometimes a fasting glucose, but rarely includes fasting insulin, a full androgen panel, SHBG, or an LH to FSH ratio, the markers most relevant to a PCOS evaluation described in Section 7. A normal fasting glucose specifically can be reassuring on the surface while still missing significant insulin resistance, since blood sugar can remain normal for years while insulin climbs to compensate, which is exactly why fasting insulin, not fasting glucose alone, is central to an accurate evaluation.
I've had normal periods my whole life. Could I still have PCOS?
It is less likely, since irregular or absent ovulation is one of the three Rotterdam criteria, but it is not impossible. A small subset of women with PCOS ovulate regularly enough to have largely predictable cycles while still meeting the other two criteria, excess androgens and the ovarian ultrasound finding.
